Any patient who does not have dental insurance can purchase a membership. Memberships are not considered insurance. All dental care must be provided by Kewaskum Family Dentistry. Membership benefits are not transferable to another dental office or person. It is the member’s responsibility to schedule and keep all appointments that are offered through this program. Memberships are good for one year from the date of purchase. All benefits must be used within that time period. Any unused services cannot be carried over to the new year. Memberships begin on the date of purchase. Membership fees are to be paid in full at the time of enrollment and are non-refundable. If memberships are not paid in full, half down will need to be paid at the time of purchase followed by 2 monthly installments for complete payment of membership. These membership payments can be in the form of cash, check or credit card. No additional discounts apply to membership fee.
